Since January, providers have been struggling to reconcile conflicts between CMS' rules and regulations and those published by the CPT® Manual and other AMA publications.
In addition to confusion over billing for clinical laboratory tests and issues related to observation status and the 2-midnight criteria, providers have also been grappling with several contradictions between CMS and the AMA.
