Can I use the "Prolapse of vaginal vault after hysterectomy" code if the patient has a prolapse and had a hysterectomy in the past without the physician specifically saying those words? I just wonder if it is an automatic versus me having to ask the physician to document that exactly in one sentence. Thanks!
