When counting HPI elements, do negative responses count? For example, "There are no alleviating or modifying factors" or "Associated signs and symptoms - none." Where can I find in writing whether credit can be given for a response that equal none? My physicians are telling me that this information is clinically relevant to them and shows the question was asked and answered, but I was not trained to allow credit for "none" for an HPI element. Thank you.
