A: Query the physician to determine whether acute renal failure was POA. Appendix I of the ICD-9-CM Official Guidelines for Coding and Reporting (POA Reporting Guidelines) indicates that a query is appropriate if documentation is unclear regarding whether a condition was POA. The provider should clarify the linkage of signs and symptoms to the acute renal failure, the timing of test results, and the timing of findings.
Editor’s note: Laura Legg, RHIT, CCS, revenue control coding consultant at Providence Health & Services in Renton, Wash., answered this question, which originally appeared in the June issue of Briefings on Coding Compliance Strategies.
