CMS added four new J codes for reporting drugs and biologicals that previously did not have specific codes available as part of the 2012 Outpatient Prospective Payment System updates (Transmittal 2376).
The new J codes are:

  • J0257, injection, alpha 1 proteinase inhibitor (human), (glassia), 10 mg
  • J7180, injection, factor xiii (antihemophilic factor, human), 1 iu
  • J7326, hyaluronan or derivative, gel-one, for intra-articular injection, per dose
  • J8561, everolimus, oral, 0.25 mg

In addition, CMS added four other codes for drugs and biologicals:

  • A9585, injection gadobutrol, 0.1 ml
  • C9287, injection, brentuximab vedotin, 1 mg
  • C9366, EpiFix®, per square centimeter
  • Q4122, Dermacell®, per square centimeter

Coders should also review the changes to code descriptors for drugs and biologics included in Table 5 In Transmittal 2376. CMS also deleted several temporary HCPCS C-codes effective December 31, 2011, and replaced them with permanent HCPCS codes in calendar year 2012.
