Coders and other HIM staff members can do several things to ensure that they report excisional debridement correctly. One is reviewing the relevant coding guidelines.

Gloryanne Bryant, BS, RHIA, RHIT, CCS, CCDS, regional managing director of HIM (Revenue Cycle N. California) for Kaiser Permanente in Oakland, CA, provided the following list of AHA Coding Clinic references that coders and others may find particularly helpful:
  • First quarter 2008, “Wound debridement of coccyx,” “Excisional debridement documented,” and “Excisional debridement”
  • Second quarter 2005, “Excisional debridement of sites not listed in index”
  • First quarter 2003, “Outpatient observation for monitoring/dressing changes after MVA”
  • Second quarter 2000, “Excisional debridement of wound”
  • Third quarter 1991, “Excisional vs. nonexcisional debridement—guideline”
  • Fourth quarter 1988, “Debridement of wound, excisional vs. nonexcisional—guidelines”
