A physician provided the following documentation for an injection: “Cervicothoracic bursal injection under fluoro.” The dictation states: “We used a 27-gauge needle and advanced it to the intermediate ligament just outside the ligamentum flavum. We injected between the intermediate and ligamentum flavum using 3 cc of 1/4 percent Marcain and 10 milligrams of Depo-Medrol.” What CPT code(s) should we report?
