Q&A: Coding for sclerotherapy of vulvar varicosities

QUESTION: Which code should I use to report sclerotherapy of vulvar varicosities that the physician performs with a direct injection of Sotradecol® for a total of three injections?

ANSWER: The CPT Manual’s Alphabetic Index identifies sclerotherapy, venous and guides coders to codes 36468–36471. The term “sclerotherapy” indicates the injection of solution. The treatment for this condition of the veins is to inject special solution into the affected vessel.

The vulva is technically a part of the body’s trunk. My initial research indicates that this is the same therapy physicians use to treat the same condition in the lower extremities. Report code 36468 (Single or multiple injections of sclerosing solutions, spider veins [telangiectasia]; limb or trunk) only once because the description reports single or multiple injections.
