Consider using HHS' online breach notification forms

HHS unveiled its online breach reporting forms one week after its interim final rule on breach notification became effective September 23. The forms ended speculation about how HHS wants covered entities to submit breach notifications to fewer than and more than 500 victims.
The forms, which are available at http://transparency.cit.nih.gov/breach/index.cfm, are not as long, detailed, or time-consuming as providers may have expected. Instead, the format of the online forms is the same for:
- Reporting breaches with fewer than 500 victims
- Reporting breaches with more than 500 victims
- Submitting an initial breach report
- Submitting an addendum to a previous report
The sections are concise and direct; covered entities likely will need only a few minutes to complete the forms and send them to HHS.
