Forms available for breach notification

On Oct. 1, the Department of Health and Human Services (HHS) released forms to submit information to HHS regarding unsecured medical information breaches under the breach notification interim final rule.
According to the American Hospital Association, providers must notify the HHS secretary by submitting the breach report form. If the breach effects 500 or more patients, the provider must notify the secretary concurrently with the individual notices to affected individuals. If the breach effects fewer than 500, they may notify the HHS on an annual basis, but no later than 60 days after the end of the calendar year.
