Survey watch: H&Ps continue to cause headaches for HIM directors

Obtaining a timely history and physical (H&P) remains a constant challenge for most HIM directors, as evidenced by the volume of questions that “MRB Talk” subscribers regularly post on this topic.
The Joint Commission (formerly JCAHO) has posted FAQs about H&Ps on its Web site, most recently in February, but confusion and noncompliance persist.
