CMS pilot program targets hospitals readmissions

On April 13, CMS announced a pilot project to reduce rates of hospital readmissions. The Care Transitions Project will run through the summer of 2011 and includes 14 regions that will work to promote seamless transitions from hospitals to homes, skilled nursing care facilities, or home health care. The regions include:

  • Providence, RI
  • Upper Capitol Region, NY
  • Western Pennsylvania
  • Southwestern New Jersey
  • Metro Atlanta East, GA
  • Miami
  • Tuscaloosa, AL
  • Evansville, IN
  • Greater Lansing Area, Michigan
  • Omaha, NB
  • Baton Rouge, LA
  • Northwestern Denver, CO
  • Harlingen, TX
  • Whatcom County, WA 

A state quality improvement organization will lead the effort in each of the regions to implement:

  • Hospital and community system-wide interventions
  • Interventions that target specific diseases or conditions
  • Interventions that target specific reasons for admission
