Coding Shone's syndrome with subaortic stenosis and coarctation

Q: How should I code Shone’s syndrome with subaortic stenosis and coarctation? Should I report code 746.84 for Shone’s syndrome and code 396.2 (mitral valve insufficiency and aortic valve stenosis) for the subaortic stenosis and coarctation?

A: The code for Shone’s syndrome in the tabular index of the 2009 ICD-9 Manual state, "Use an additional code for associated anomalies, such as coarctation of aorta (code 747.10), congenital mitral stenosis (code 746.5), and subaortic stenosis (code 746.81)." In the alphabetic index, you’ll find "stenosis, subaortic," which includes code 746.81. When searching for "coarctation, aorta," you’ll find code 747.10. Therefore, assign codes 746.81 and 747.10 for subaortic stenosis and coarctation, respectively.
