Boulder Community Hospital has notified police that copies of patient intake forms are missing—but it doesn’t know whether the paperwork was stolen or simply misfiled, the Boulder, CO Daily Camera reported September 4.
Paperwork for as many as 178 of the 207 patients who visited the Colorado hospital’s occupational health and therapy services department between April and June is missing, according to the article. The hospital notified police because they are unable to confirm that the patient information wasn’t stolen.
“We are taking this very seriously, and we’re very sorry that this occurred,” hospital spokesman Rich Sheehan told the Daily Camera.
