Tip of the week: Don't report an E/M with every procedure; follow AMA guidelines for modifier -25

You should not charge or code an E/M each and every time you perform a wound care procedure. An OIG report expressed concern about this practice. The E/M must be significantly separate from the procedure. Follow AMA guidelines with respect to using an E/M with a procedure and assign modifier -25 to the E/M.
