The 2008 OPPS final rule challenges outpatient facilities to be more exact than ever regarding documentation and coding. This is most true when it comes to the E/M process. Hospitals run into difficulty putting the rule into practice, shedding some cherished but inaccurate beliefs about documentation best practices, and widening the circle of responsibility for documentation improvement.
William L. Malm, ND, RN, practice director of revenue cycle management at HCPro, Inc., in Marblehead, MA, and Shelia Gunn, CPC, CCS-P, PCS, CMBS-I, president of Medical Coding Consulting in Midlothian, VA, spoke about these problems at a recent HCPro audioconference, “Facility E/M Update: Meet CMS’ Latest Coding and Documentation Requirements.” They also reviewed what the new requirements mean to facilities, and ways to manage them.
