PPV: Solicit detailed documentation to improve your hospital's profile for stroke patients

It has been quite a while since I’ve written about stroke documentation. Since my last column on this topic, more and more facilities have grown concerned with their profiles regarding stroke patients. This is because DRG 14 (Specific cerebrovascular disorders except transient ischemic attacks) has now become DRGs 61–66 under Medicare Severity DRGs (MS-DRG).

The first three MS-DRGs (61, 62, and 63) represent ischemic strokes with use of thrombolytics. CMS divides these DRGs into “with major complication and comorbidity” (MCC), “with CC,” and “without CC or MCC.” The second three MS-DRGs (64, 65, and 66) represent cerebral bleed and ischemic strokes, but without use of thrombolytics, and are also divided into three subgroups.
