Wound care coding and billing: Address compliance risks

wound care. when to code injury and when to code follow up and ulcer. for debriment done on visit. documentation saying ( injury to leg.but it happens months ago. and is current coming for debriment. but dianosis is saying injury. should we code injury or should we code, ulcer. to me knowledge we should code to ulcer (wound). thanks
