Two computers were stolen the night of November 6 from a health center in Jefferson, IN, according to a November 25 article in the Evansville, IN Courier & Press. The health center was contracted with the Indiana Department of Health to manage information for the state's Breast and Cervical Cancer Program.

The computers contained password-protected PHI, including names, addresses, birthdates, and Social Security numbers, as well as medical and billing information, for more than 7,500 Indiana women, according to the article.

The Indiana health department notified the women whose information was contained on the computer via a letter sent within a week of the theft, according to the Courier & Press.
