Documentation is a team sport

Homecare Insider, June 27, 2016

Documentation of terminal illness at admission and during the hospice service period must be detailed and valid, whether it’s electronic or on paper. Every member of the interdisciplinary team (IDT) has a role to play. If you’re struggling with documentation, tune in July 12, from 1:00 p.m. to 2:30 p.m. Eastern, for a webcast titled “Hospice Documentation: Identify Risk Areas to Reduce Regulatory Scrutiny.” During this presentation, speaker Jennifer Kennedy, MA, BSN, RN, CHC, will drill down into Medicare’s documentation requirements to help you identify your hospice’s risk areas. Attendees will also learn how IDTs can proactively document to consistently show why patients are hospice-eligible, and develop strategies for creating compliant documentation.
