For hospitals, The Joint Commission standards are clear regarding what type of information the medical staff must evaluate prior to granting privileges. Your organization must evaluate the following:

  • Any previous or current challenges to the practitioner's license or registration
  • A practitioner's voluntary or involuntary relinquishment of his or her license or registration
  • A practitioner's voluntary or involuntary termination of his or her medical staff membership
  • A practitioner's voluntary or involuntary restriction of clinical privileges, including the limitation, reduction, or loss thereof
  • Evidence of unusual patterns or a high number of professional liability actions that result in final judgments against the applicant
  • Documentation of the applicant's health status

More information can be found in The credentialing coordinator's handbook, by Anne Roberts, CPMSM, CPCS, available here.
