According to the National Practitioner Data Bank's (NPDB) fact sheet, a hospital or other healthcare entity "must report adverse actions within 15 days from the date the adverse action was taken or clinical privileges were voluntarily surrendered."

Note: Individual states may have more stringent reporting requirements (see "New California law will affect reporting" in this issue); you must also make sure your organization's reporting practices are in compliance with both state and federal law.
