Tip of the week: Gather information from outside sources at reappointment

While the burden of providing adequate information in a timely fashion is the responsibility of the practitioner during the reappointment process, it is not the only data you will need for reappointment.
Gather as much data as you need from outside sources. Such information includes, but is not limited to, the following:
- Patterns of care, as demonstrated in findings of quality/utilization monitoring activities
- Medical records/hospital reports
- Continuing education activities (if any)
- Attendance at required medical staff meetings (if any)
- Service on medical staff committees
- Timely and accurate completion of medical records
- Compliance with medical staff bylaws and all applicable policies, rules, regulations, and procedures
- A current National Practitioner Data Bank report
- Information from the Office of Inspector General's (OIG) List of Excluded Individuals and Entities (Medicare and Medicaid)
