Ask the Expert: What information should the MSO request at reappointment?

At least 60 days prior to the expiration of a practitioner's medical staff appointment, your medical staff office should require the following if he or she is requesting privileges:
- Complete information to update his or her original application
- A specific request for any modification of his or her clinical privileges, accompanied by information demonstrating current clinical competence in the privileges requested
You should provide the practitioner with a new request for clinical privileges and a list of his or her current privileges, taken from the existing credentials file. Request that the practitioner complete the new privilege request form by reviewing the list and making any necessary changes.
Remember, the practitioner should have the burden of producing adequate information and resolving doubts about questionable data.
