The Joint Commission requires that certain criteria are evaluated before a hospital can grant, renew, or revise a physician's privileges. While hospitals may add additional criteria at their own discretion, the following must be evaluated:

  • Challenges to Licensure
  • Voluntary and involuntary relinquishment of license or registration
  • Voluntary or involuntary termination of medical staff membership
  • Voluntary and involuntary limitation, reduction, or loss of clinical privileges
  • Evidence of an "unusual pattern or an excessive number of professional liability actions resulting in a final judgment" against the applicant
  • Documentation of the applicant's health status
  • Relevant practitioner-specific data-compared to aggregate data when available
