Medical record completion is not a new problem for hospitals. For decades, medical staffs often treated medical record completion as having little, if any, obvious consequence. Today, however, the consequences for allowing incomplete medical records are substantial. In addition to jeopardizing hospital patient care, incomplete records cause hospitals to face significant opportunity costs for providing unbillable care.

Perhaps even more importantly, medical record completion affects corporate compliance. The federal government is now putting more pressure on hospitals to bill only for services that are accurately and adequately documented and justified within the patient record. The need for a policy addressing medical record completion is obvious.
