The World Health Organization Collaborating Center on Patient Safety (Solutions), the World Alliance for Patient Safety, and the Commonwealth Fund announced this month announced a seven-country collaborative project intended to push for the implementation of patient safety solutions to prevent catastrophic events in hospitals, according to an official release from the Joint Commission on Accreditation of Healthcare Organizations.

The solution areas at the heart of this collaborative effort are:

  • Prevention of patient care hand-over errors
  • Prevention of wrong site/procedure/person surgical error
  • Prevention of continuity of medication errors
  • Prevention of high concentration drug errors
  • Promotion of effective hand hygiene practices

The partnership between Australia, Canada, New Zealand, the United Kingdom, and the United States of America hope to make significant reductions to these five extremely prevalent patient safety problems over a five-year period.
