JCAHO calls for more accurate medication reconciliation

The Joint Commission for Accreditation of Healthcare Organizations (JCAHO) issued a statement this week urging greater attention to medicines given to patients who are transferred between healthcare locations or between physicians.

 

According to the JCAHO's statement, United States Pharmacopeia received more than 2,000 voluntary reports of medication reconciliation errors. More than 7,000 deaths occur each year in hospitals alone due to medication errors, according to a 1999 survey by the Institute of Medicine.

 

A medicine reconciliation error occurs when patients are given incompatible drugs, wrong dosages, or wrong dosage forms, among other errors.

 

The JCAHO called for a "systematic approach to reconciling medications," said JCAHO president Dennis S. O'Leary, MD.
