Dear credentialing colleague:

Your hospital has received an application from a 54-year-old general surgeon. This physician attended a well-regarded medical school, and successfully completed a residency program more than two decades ago. For the past 15 years, he has been the sole general surgeon at a very small hospital in rural America. He has had no disciplinary actions against him, and no malpractice suits. He is fully licensed, has a valid Drug Enforcement Administration (DEA) registration and has excellent references from the administrator of his past hospital, a family physician, and an internist. He has decided to relocate to your community for lifestyle reasons and is requesting general surgical privileges.

How would you handle this application? (Whether you use a laundry list or core privileging system is not pertinent for purposes of this quiz).
