Hospitals in rural areas face some formidable peer review obstacles. For starters, rural hospitals might not have a large enough medical staff to support objective peer review because every physician is either a partner, competitor, relative, or friend with the next physician. In addition, there may not be another physician in the same specialty as the physician under review to provide clinical expertise.
