Ask the expert: What are some examples of prospective proctoring that can be used during focused professional practice evaluations?

There are several types of prospective proctoring. The prospective proctoring for cognitive diagnostic/medical evaluation is best accomplished through an early discussion between the newly-privileged applicant and the proctor. A sample scenario would be: A newly privileged family practitioner or internist has just seen a patient in the emergency room presenting with chest pain. The new practitioner would call the proctor, describe the physical findings and laboratory results with the proctor, and outline his or her plans for therapeutic management of this patient (e.g. admission to the intensive care unit for further monitoring and/or cardiology consult). The proctor would then document the interaction following further evaluation of the patient and/or patient’s chart, as indicated.

The prospective proctoring for procedural/surgical evaluation would be accomplished through a pre-procedure conversation with the newly-privileged practitioner and the proctor, similar to the scenario above. A sample scenario in this case would be: A newly privileged general surgeon evaluated a patient in the emergency room and had a preoperative diagnosis of a bowel obstruction necessitating the patient being taken to the operating room. The new practitioner would call the proctor, describe the findings to the proctor, and outline the surgical management of the case. The proctor would then document the interaction following further evaluation of the patient and/or patient’s chart as indicated.
