On request, the medical staff services department should provide prospective applicants an application packet that includes the following:

  • A blank application form with a cover letter outlining membership eligibility criteria
  • A list of required supporting information the applicant must provide
  • A list of performance expectations for individuals granted medical staff membership and/or privileges (if such a list of expectations has been formally adopted by the medical staff)
  • A description of medical staff member responsibilities
  • An overview of the delineation of privileges
  • Privileges request forms, including criteria for privileges
  • A detailed list of requirements for completion of the application

This week’s question and answer are from The Greeley Guide to Medical Staff Bylaws, Second Edition, by Joseph D. Cooper, MD, CMSL.
