MSPs should present the practitioner’s updated quality file to the department chief at the time of reappointment. The quality file should contain all peer review information, all citizenship data, and all ongoing review data. Also include a two year quality profile that gives the chief a quick glimpse of the applicant’s performance over the past two years. It is essentially a summary of what should already be in the quality file based on the ongoing review.
The chief should take into consideration all the information contained within the quality file when making his or her decision to grant the practitioner privileges and continue membership at the time of reappointment. The chief may also decide, based on the information, that the applicant needs closer monitoring and recommend that the applicant’s reappointment be granted on a conditional basis so long as he or she successfully completes a focused professional practice evaluation.
