Free form: Letter denying application request

On certain Fridays, the Credentialing Resource Center (CRC) Blog publishes a free credentialing form. Last week Associate Editor Emily Berry posted a sample letter denying a request to process an application for medical staff membership. The form can be customized to deny a request for privileges, or it can be edited to include your organization’s criteria.
