Welcome back to the second week of Free Form Friday. This week’s form is a sample letter denying a request to process an application for medical staff membership. The form can be customized to deny a request for privileges, and it can be edited to include your organization’s criteria.
To access this week’s form, log onto the Credentialing Resource Center blog at www.credentialingresourcecenter.com/blog, and scroll down to the post “Free Form Friday: Letter denying application request.”
While you’re on the blog, be sure to check out other posts, including, “Breaking News: Joint Commission adjusts medical staff standards to comply with CMS regulations.”
What free form would you like to see next week? Drop me a line at eberry@hcpro.com and let me know.
Cheers!
