Q&A: How do I verify military service longer than 10 years ago?
Credentialing & Verification Update, November 18, 2008
Military service can often be especially challenging to verify, especially if the service occurred more than 10 years ago or involved work at a wartime hospital that no longer exists.
Whenever possible, military service should be verified directly with the hospitals provided by the applicant or via professional references. If this is impossible, you can verify military service by contacting the National Personnel Records Center by fax at 314-801-9195 or by mail at:
9700 Page Avenue
St. Louis, Missouri 63132
Bu sure to include the following information with your request:
- The veteran's complete name used while in service
- Service number or social security number
- Branch of service
- Dates of service
- Date and place of birth may also be helpful, especially if the service number is not known
