New sample form available on CPDR Online
Credentialing & Verification Update, September 9, 2008
A new credentialing form is available for download at the Credentialing and Privileging Desk Reference (CPDR) Online.
The completely customizable Microsoft Word form, a sample medical staff application for intitial appointment, is available for download in the verification requests form section of CPDR.
