New sample form available on CPDR Online
Credentialing & Verification Update, September 9, 2008
A new credentialing form is available for download at the Credentialing and Privileging Desk Reference (CPDR) Online.
The completely customizable Microsoft Word form, a sample medical staff application for intitial appointment, is available for download in the verification requests form section of CPDR. Access to this and other forms is limited to CPDR subscribers and trial participants. If you are not yet a CPDR subscriber, enroll in a free trial of CPDR now to get access to this resource.
