Q&A: What information must be verified at reappointment?

At the time of reappointment, the MSP does not need to go back and reverify static information, i.e., information that has not changed since your organization previously verified it. The only information that needs to be verified is anything that has changed between the practitioner's last reappointment or initial appointment and anything that is current. The following is a sample of the type of information your organization should verify to ascertain whether it is current or whether the practitioner relinquished such affiliation/license since he or she was last credentialed by your organization.
- All organizations where the practitioner currently holds membership or privileges, or any memberships or privileges that he or she has relinquished (voluntarily or involuntarily)
- All current state licenses or any that the practitioner has applied for or relinquished (voluntarily or involuntarily)
- Current academic appointments or any that the practitioner has relinquished (voluntarily or involuntarily)
- Any insurance companies that have provided coverage to the applicant
