Report links deaths at VA hospital to substandard care in several areas, including negligent credentialing

The medical inspector of the Veterans Health Administration issued a report on January 28 citing substandard care as the cause of nine patient deaths and 34 other serious injuries at the Marion (IL) Veterans Affairs Medical Center, according to a January 29 Chicago Tribune article.
"Ten of the 34 injured patients later died, but investigators were unable to determine if the substandard care they received at the medical facility was the cause," according to the article.
The substandard care at the medical center, according to the article, included:
- Hiring physicians without fully checking their credentials
- Allowing surgeons to perform procedures for which they were not trained
- Failing to act on information that suggested that patients were at risk
