Military training and/or work history can be verified by completing the "Request Pertaining to Military Records" form that MSPs can print from the following Web site:
http://www.archives.gov/research/order/standard-form-180.pdf
If you do not know what branch of the service the provider was part of, one way to find out is by calling the Army Reserve Personnel Command at 314/592-0000, extension 3723 and they can answer your question, as well as provide you with more information about the request form. You will need to have the provider's social security number available at time of the call.
