Medical staff offices should regularly audit themselves to avoid the two most common types of credentialing errors:

  • Information error: Information existed that could have been known but wasn't, and the information would have had an effect on the credentialing decision.
  • Decision error: Hospital leaders knew the necessary information, but they failed to make a wise decision based on that data.

Tips for conducting audits can be found in Credentialing Audits: Tools for Compliance and Reduced Liability, by Vicki L. Searcy, CPMSM, available here.
