Tip of the Week: Avoid information and decision errors during the credentialing process

Medical staff offices should regularly audit themselves to avoid the two most common types of credentialing errors:
- Information error: Information existed that could have been known but wasn't, and the information would have had an effect on the credentialing decision.
- Decision error: Hospital leaders knew the necessary information, but they failed to make a wise decision based on that data.
