"If physicians do not level their own E/M services in the outpatient setting but do on the inpatient setting, they may not have enough education about the documentation requirements for each service," says Joe Rivet, CPC, CCS-P, revenue manager in the department of internal medicine for the Detroit-based Henry Ford Health System.
