Physicians are overloaded with tasks-from staying on top of the latest medical advances to breaking bad news to patients. So it's no surprise they need a little help when it comes to evaluation and management (E/M) coding. "If physicians do not level their own E/M services in the outpatient setting but do on the inpatient setting, they may not have enough education about the documentation requirements for each service," says Joe Rivet, CPC, CCS-P, revenue manager in the department of internal medicine for the Detroit-based Henry Ford Health System.

"Anytime you have someone selecting a CPT or ICD-9 code who is not familiar with coding conventions, there is an increased risk of selecting the wrong code," says Rivet.
