Tip of the week: Ask why

During an audit, identifying the root cause of a good or bad performance is key. The auditors should repeatedly probe the issue by asking, Why? For example, it might be apparent that employee error led to the condition, but by asking why, auditors may find this immediate cause is not the root or underlying cause. The auditors might find the employee's action was due to inadequate training. By asking why the employee's actions were incorrect, the team may determine that management failed to undertake an important training program, which would mean that a lack of proper training was the root cause of the condition.Knowing why a condition occurred is essential to developing meaningful and creditable recommendations in the audit that will both correct the problem and be acceptable to management.
