Clarification of covered ICD-9-CM codes for percutaneous transluminal angioplasty (PTA)
CMS is amending the Medicare Claims Processing manual to add ICD-9-CM diagnosis code 433.11 (occlusion and stenosis of carotid artery, with cerebral infarction) to the list of covered diagnosis for PTA of the carotid artery concurrent with placement of an FDA-approved carotid stent.
Effective Date: March 17, 2005
Implementation Date: October 1, 2007

Balance billing limitation on services furnished to Indian health service patients
CMS has issued a "Special Edition" MLN Matters article addressing certain balance billing limitations applicable to services furnished to patients covered under the Indian health services and certain other tribal and urban Indian organization programs.
