Silos characterize care delivery in the United States. Primary care physicians often are the first point of entry into the system. They frequently send patients to reference labs, imaging centers, or other providers or hospitals for diagnostic studies. They may refer patients to specialists. They admit patients to hospitals for major surgery, or for treatment by hospitalists and others who specialize in acute-care delivery for ill patients. A patient may transfer to a rehab facility, long-term care center, or home health agency in a follow-up to his or her discharge from a hospital. Patients may or may not return to their primary care physicians. Even if patients want to return to their primary care providers, a change in their health plan benefits may preclude this, or the physician may have left the practice-especially as increasing numbers of physicians are now employed by groups.
