Verifying the identities of public officials

How can we verify public officials' identities when they request PHI?
You can rely on any of the following to verify identity when disclosing PHI to a public official or a public official's representative:
- An in-person request accompanied by an agency identification badge, official credentials, or other proof of government status
- A written request on appropriate government letterhead
- A written statement on appropriate government letterhead that a representative is acting under the government's authority, or other documentation (e.g., a contract for services, memorandum of understanding, or purchase order) that establishes that the person is acting on behalf of the public official
