Tip: Using modifier -74

Use modifier -74 when, due to extenuating circumstances, the physician cancels a surgical or diagnostic procedure after the administration of anesthesia or after the procedure was started.
The following are tips for using modifier -74:
The patient has to be taken to the room where the procedure is to be performed to report modifier -74
Do not report the elective cancellation of a service before the administration of anesthesia or surgical preparation of the patient
When one or more of the planned procedures is completed, report the completed procedure as usual; do not report the other procedures that were planned but not started
When none of the planned procedures are completed, report the first procedure that was planned to be performed with modifier -74; do not report the other procedures
