When authorizing requirements for HIPAA uses and disclosures, guarantee compliance by ensuring that

  • the organization's authorization form contains all the valid elements and that it accepts only authorizations from others with all valid elements

  • the organization obtains an authorization from the individual when required by HIPAA, state statute, and organizational policy

  • the organization verifies the identity and authority of any person or organization to whom protected health information is disclosed when an authorization is not required
