Weigh in on a recent MedicareFind Forum post:
How should we bill Medicare for clia waived tests in a provider based clinic? Should it be billed on the 1500 or on the UB? If billed on the 1500 should the place of service be an 11 or a 22? If billed on the UB, do we need the QW Modifier. What transmittals or documents would help me answer these questions?
